Overview

VNA Health Group is a premiere Medicare/Medicaid-certified provider of home health, hospice, and community-based services. We’ve been serving families and individuals with the most advanced, comprehensive clinical home care in NJ since 1912.

 

We are currently looking for a Staff Nurse to join our team. This position would be responsible for providing skilled nursing care to patients with life threatening and life limiting illnesses and their families. Will ideally work 10am-8pm Tuesday-Friday and a one weekend per month committment.

Responsibilities

  1. Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  2. Assesses and evaluates the needs of patients with life threatening and life limiting illnesses and their families and develops an appropriate plan of care
  3. Identifies teaching needs and plans appropriate strategies for individuals and families
  4. Monitors quality and appropriateness of services provided
  5. Collaborates with patient and family, other members of the disciplinary team and other health care providers in providing patient and family care
  6. Documents all changes in patient’s condition and updates care plans and outcomes
  7. Understands CMS Conditions of Participation
  8. Adheres to agency infection control and safety policies
  9. Adheres to the organization’s policy in regards to absenteeism and appearance

 

Qualifications

  1. RN with Bachelor’s degree in Nursing
  2. CHPN required
  3. 1+ year of supervised clinical experience as a registered nurse
  4. Excellent communication skills
  5. Strong Computer skills
  6. Excellent organizational and interpersonal skills

Working Conditions/Physical Demand: Working indoors/outdoors with clinical equipment and use of phone and computer; car and valid NJ Driver’s license
