Overview

VNA Health Group is a premiere Medicare/Medicaid-certified provider of home healthhospice, and community-based services. We’ve been serving families and individuals with the most advanced and comprehensive clinical home care in NJ since 1912. 

 

We are currently looking for a Community Health Nurse to join our GNHCC team (Greater Newark Health Care Coalition). The position would be responsible for providing health promotion and prevention activities and referral services at Faith in Prevention Wellness sites focusing on preventing, detecting, educating, and following-up to assist community members  in achieving and maintaining their optimal level of functioning. 

Responsibilities

  1. Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  2. Attends weekly team meetings, case conferences and clinical supervision meetings 
  3. Assesses and evaluates needs of FIP participants and their families
  4. Manages a caseload and completes home visitation in accordance with Faith in Prevention model
  5. Provides counseling and instruction to assist FIP participants and their families
  6. Develops and maintains community relationships to support client referrals
  7. Adheres to agency infection control and safety policies
  8. Adheres to the organization’s policy in regards to absenteeism and appearance
  9. Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position

Qualifications

  1. Current NJ RN licensure
  2. 2+ year supervised clinical experience as a registered nurse
  3. Experience working with a diverse population and community services
  4. Experience with developing and implementing community education programs
  5. Car and a valid N.J. driver’s license
  6. Excellent verbal and written communication skills
  7. Experience and competency with use of computers, Microsoft Office and clinical software
  8. Strong organizational and interpersonal skills 

Working Conditions/Physical Demand: Working outdoors and indoors with phone, computer and medical equipment use
