  • Overview

    Responsible for providing supervision to case manager and patient navigators, oversee CHC grants and community outreach efforts and events.



    1. Provides supervision and oversight of CHC grant and patient navigators/certified application counselors
    2. Provides oversight to CHC grants funded both locally, privately and at the state level (does not include oversight of federal grants)
    3. Develops new grant proposals including program, budget, and evaluation plan
    4. Coordinates community outreach and participates in community events, programs and coalitions
    5. Conducts and attends annual CHC community events and those outlined by CHC grants
    6. Coordinates and supervises CHC Volunteer Program


    1. Master’s degree required; MPH preferred, MSW considered
    2. Strong quantitative and writing skills required
    3. CHES a plus
    4. 2-3 years related work experience
    5. Proficiency in advanced office skills
    6. Strong computer skills
    7. Excellent organizational and communication skills
    8. Excellent interpersonal skills
