    Position Summary:

     

    Responsible for generating hospice business and cultivating current relationships, providing care coordination and collaborating with physicians, hospitals and facilities, health care providers and others in the community as well as supporting and promoting new programs and service lines and works to enhance positive patient and community experience. 

    Responsibilities

    1. Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
    2. Assists to develop strategic relationships and partnerships with key health care and community leaders and physicians to develop new programming and generate new business.
    3. Responsible for generating clinically appropriate referrals and admissons for hospice
    4. Performs hospice and goals of care conversations with patients and families when appropriate
    5. Acts as a contract for key strategic partners, facilities and other referral sources
    6. Works with internal departments to generate ideas and develop joint strategies for new business and programming
    7. Ensure compliance with regulatory and statutory requirements
    8. Participates in the creation of content value propositions, solutions, messaging, datasheets aimed at communicating the value of VNAHG's services to healthcare customers
    9. Monitors performance and evaluates the effectiveness of business development strategies
    10. Analyzes data and prepares reports for business development activities and initiatives
    11. Adherest o the organization's policy in regards to absenteeism and appearance

    Qualifications

    1. RN Required
    2. BSN or CHPN Preferred
    3. Experience in oncology or hospice preferred
    4. Car and valid NJ Driver's license
    5. Excellent verbal and written communication skills
    6. Must have outstanding presentation skills and ability to present to large groups of people
    7. Experience and competency with use of computers, smart phones, Microsoft Office and clinical or sales software
    8. Excellent organizational and interpersonal skills

    Working Conditions/Physical Demand: Working out in the field with heavy driving and phone and computer use
