  • Overview

    Working directly with the Patient Experience Team to uphold the culture and values of the VNAHG, this person and family centered position, will be responsible for advocacy on behalf of patients and their families to provide support as needed and ensure a positive patient experience. Working with clinical staff to speak with patients who have voiced a concern and provide feedback   on areas of improvement for patient experience and service excellence. This role will develop a relationship of trust with patients and families, facilitate proactive service recovery to enhance the perception of VNAHG and contribute to improved experiences of care which embody the highest degree of service and compassion.


    Must be flexible with travel.


    1. Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
    2. Communicates patient preferences of care for service recovery plan, in partnership with staff, patient, representative (if any) and caregiver(s) acting as a liaison for specific service recovery efforts
    3. Work in partnership with the managers and staff to resolve issues or complaints with early intervention and problem resolution
    4. Assess and help resolve patient and family issues that create challenges with receiving appropriate care from VNAHG
    5. Supports Department of Patient Experience efforts for continuous improvement through Service Excellence Standards
    6. Adheres to the organization’s policy and procedure on documentation
    7. Adheres to agency infection control and safety policies
    8. Adheres to the organization’s policy in regards to absenteeism and appearance


    1. Master’s Degree in Social Work accredited by Council on Social Work Education required with current NJ LSW/ MSW
    2. 1+ year of supervised clinical experience in a healthcare setting; community health experience preferred
    3. Patient and Family Advocacy or Customer Service experience preferred
    4. Excellent verbal and written communication skills
    5. Excellent interpersonal skills
