    We are currently looking for a Field Manager to join our team. This position would be responsible for planning, directing and supervising the quality of care provided by the Home Health Nurses and providing back up support to the Clinical Manager as needed.  Responsible for providing coordination of internal and external services for patients on the team.

    Responsibilities

    1. Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
    2. Facilitates and coordinates the case management, interdisciplinary care and services provided by the staff nurses to patients and families
    3. Makes field visits with Home Health Nurses to client’s homes
    4. Monitors the quality and appropriateness of services provided by the staff
    5. Provides back-up support to the Clinical Manager as needed
    6. Participates in VNAHG's QAPI (Quality Assurance Program Improvement) and in-service programs
    7. Adheres to agency infection control and safety policies
    8. Adheres to the organization’s policy in regards to absenteeism and appearance
    9. Supervises the Nursing staff in conjunction with the organization's managerial and leadership practices
    10. Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position

    Qualifications

    1. RN with Bachelor’s degree in Nursing required or in progress
    2. 2+ years of experience in community health nursing providing direct patient care
    3. Strong computer skills   
    4. Excellent organizational and interpersonal skills with ability to supervise others
    5. Excellent verbal and written communication skills; bilingual preferred 

    Working Conditions/Physical Demand: Business Office Environment with phone and computer use
