    We are currently looking for a Wellness Nurse to join our team. This position would be responsible for providing health promotion and prevention activities and referral services at Community and Employee Wellness sites focusing on preventing, detecting, educating, and following-up to assist employees/ seniors in achieving and maintaining their optimal level of functioning


    This role will require 10 HOURS AVAILABILITY PER MONTH


    1. Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
    2. Establishes and manages wellness programs at identified sites focusing on persons or groups at risk in the community/agency
    3. Develops and implements education and screening programs as appropriate
    4. Identifies clients that may be in need of VNA or other services and plans appropriate strategy, action and follow-up
    5. Completes documentation in accordance with agency guidelines and program requirements
    6. Analyzes data and prepares reports as required by the funders and/or VNAHG
    7. Develops and maintains community networks and establishes positive relationships with internal and external customers
    8. Adheres to agency infection control and safety policies
    9. Adheres to the organization’s policy in regards to absenteeism and appearance.


    1. Current NJ RN licensure
    2. 2+ year supervised clinical experience as a registered nurse
    3. Experience working with a diverse population and community services
    4. Experience with developing and implementing community education programs
    5. Car and a valid N.J. driver’s license
    6. Excellent verbal and written communication skills
    7. Experience and competency with use of computers, Microsoft Office and clinical software
    8. Strong organizational and interpersonal skills


    Working Conditions/Physical Demand: Working outdoors and indoors with phone, computer and medical equipment use
