Behavioral Health Consultant, LGBTQ/Ryan White Center

A Champion for Mental Health in the LGBTQ Community

 

This exciting and impactful position will provide behavioral health consultation as part of a primary care team focused on providing total care to an LGBTQ population in order to improve psychosocial functioning.  

Responsibilities

Among the core responsiblities of the Behavioral Health Consultant, LGBTQ role:

 

  1. Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  2. Provide comprehensive assessment and diagnosis of patients identified by the team as having behavioral health needs.
  3. Assesses for potential psychosocial barriers to adherence with follow up visits, prescribed medications, and ordered therapies or referrals
  4. Schedule full comprehensive intakes including the identification of the diagnostic criteria for gender dysphoria necessary for transgender related care, including writing letters for transgender affirming surgeries.
  5. Assist in the detection of “at risk” patients and development of plans to prevent further psychological or physical deterioration, identified through behavioral health screening, warm hand off from the PCP or other team members or formal referral from the primary care team.
  6. Assist the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease.
  7.  Provide assessment, consultation, and brief intervention for psychological/psychiatric problems and/or disorders. Provide effective treatment planning and assisting clients in successfully achieving goals.
  8. Evaluate crisis situations and apply appropriate interventions.
  9. Teach patients, families, and staff care, prevention, and treatment enhancement techniques.
  10. Monitors the site’s behavioral health program, identifying problems related to patient services and making recommendations for improvement.
  11. Facilitates appropriate referrals to psychotherapy with culturally appropriate providers.
  12. Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position

Qualifications

The ideal candidate for the Behavioral Health Consultant position will have the following qualifications:

  • Masters Degree in Social Work required with current New Jersey LCSW licensure
  • 1+ year of supervised clinical experience; community health experience preferred
  • Excellent verbal and written communication skills
  • Excellent interpersonal skills
  • Ability to work with multiethnic and diverse socioeconomic populations

Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions. Knowledge of lesbian, gay, bisexual, transgender and HIV health/mental health and substance abuse issues and concerns, including appropriate DSM V diagnosis.  Ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of mental and behavioral conditions. Should be comfortable with the pace of primary care, working with an interdisciplinary team, and have strong communication skills. Good knowledge of psycho-pharmacology

Ability to design and implement clinical pathways and protocols for treatment of selected chronic conditions. To perform this job successfully, an individual should have knowledge of Word Processing software and be comfortable with documenting and navigating electronic health records.
