Community Health Center (CHC) offers comprehensive, community based, preventive, primary care and wellness services for New Jersey residents of all ages at four convenient locations.CHC health care services are designed to meet the specific needs of the entire family – from annual check-ups and immunizations to sick visits, mental health counseling and critical prenatal care. 

 

Position Summary:

Community Health Center is seeking an Psychiatric Advanced Practice Nurse for their Monmouth County, NJ Facilities.  This position will be responsible for providing adult mental health services to primary care, prenatal & HIV clients of the VNACJ Community Health Center in Asbury Park and collaborating with the Psychiatrist for complex cases

Responsibilities

Principal Responsibilities:

  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  • Provides mental health services for primary care, prenatal & HIV clients and families within established guidelines
  • Collaborates with other health team members to develop and implement client’s psychiatric plan of care
  • Monitors quality and appropriateness of services provided
  • Initiates client teaching for individuals and families regarding the psychological aspects of illness
  • Prescribes psychiatric medications as needed
  • Participates as part of the health care team
  • Participates in community outreach to educate the community regarding CHC services
  • Adheres to agency infection control and safety policies and guidelines of primary care regulatory agencies

Qualifications

Job Competencies & Minimum Qualifications:

  1. Licensure as an Advanced Practice Nurse in NJ
  2. Master’s degree in Nursing from an NLN accredited program
  3. Certification as a Psychiatric APN
  4. Maintains current CPR and professional certifications
  5. Excellent psychiatric skills and thorough knowledge of current practice trends
  6. Strong verbal and written communication skills
  7. Excellent interpersonal skills

 

Working Conditions/Physical Demand: Working indoors.  Exposure to blood, body fluids and communicable diseases. Sitting, fine motor hand movement, standing and walking. Client/customer contact.
