At Visiting Nurse Association of Ohio, we are looking for dependable, compassionate people interested in gaining experience and becoming an invaluable resource to our organization. VNA of Ohio is now hiring a Hospice Case Manager (RN) in the Cleveland area.  Your qualifications and skill set may be an ideal match for this opportunity. This position is FT evening and on-call.

Job Summary:

Provides family-centered comprehensive care to patients of various age and diagnostic groups who require on-call intervention.   These include specialty populations such as infusion therapy, hospice and pediatrics.  The on- call support may include clinical phone guidance, coordination of care, scheduling, and direct patient intervention through visits. 

Responsibilities

Essential Functions:

Illustrative duties may include, but are not restricted to, one or more of the following:

Direct Care Role:

  • Provides direct care services to patients/families in community and institutional settings.
  • Utilizes principles of the nursing process to assist in data collection and evaluation of nursing care.
  • Utilizes effective interviewing techniques; sets limits, and collects data based on specific role.
  • Demonstrates nursing knowledge and skills in the implementation of prevention, therapeutic and rehabilitative nursing care.
  • Communicates and collaborates effectively with health care providers, third parties, patients and families based on specific role.
  • Explains processes, procedures and treatments to patient to gain cooperation, understanding, and ease apprehension.
  • Maintains awareness of comfort and safety needs of patient.
  • Observes patient, records significant conditions and reactions, and notifies supervisor or Physician of patient's condition and reaction to drugs, treatments, and significant incidents.
  • Responds to life saving situations based upon nursing standards, policies, procedures, and protocol.
  • Uses oral and written communication to convey pertinent information to members of the health care team in a timely manner.
  • Complies with company policies and procedures.
  • Completes all required forms, reports, time cards, itineraries, etc, accurately and within designated timeframe.
  • Recognizes and follows established lines of communication.
  • Performs other duties as assigned.

Indirect Care Role:

  • Maintains knowledge of Horizon computer system for care delivery applications.
  • Coordinates communications with third party payors for pharmacy, DME, and other supplies needed for the clinical plan of care.
  • Maintains knowledge of regulatory, licensing and accreditation agencies.
  • Performs other duties as assigned.

Qualifications

Qualifications and Requirements:

  • Completion of an accredited RN education program with a diploma, associate or bachelor's degree. Bachelor's degree in Nursing preferred.
  • 1 year community health experience with diverse populations. Specialty experience with hospice, palliative care, and infusion therapy preferred. Pediatric experience helpful
  • 1 year experience with providing phone support and/or triage intervention in community health, clinic setting, and/or emergency room preferred.

·        Experience working as part of a team and working effectively with persons and communities from diverse cultural, social, and ethnic backgrounds. The ability to work in a complex institutional structure requiring significant collaboration. 

·        Strong planning and organizational skills.  

·        Demonstrated ability to work well under pressure, within strict time parameters, and with a strong attention to detail.

·        Ability to work independently with minimal supervision.

·        Ability to work seven days on, seven days off 4:30 p.m.-midnight, on-call between midnight-8:30 a.m. with weekend coverage between 8am -5pm Saturday and Sunday. Occasional overtime may be required to ensure adequate staff coverage.

·        Demonstrated ability to maintain strict confidentiality.

 

Required Licenses, Registrations and/or Certifications:

  • Registered Nurse; licensed in the State of Ohio.
  • CPR certification through the American Heart Association is required.
  • Valid driver's license.
