Overview

We celebrate over 120 years of service. Our not-for-profit organization has grown as VNA of Ohio and serves thousands of patients and clients across the Northeast and Mid-Ohio region.

 

VNA of Ohio currenlty has an opening for Patient Access LPN (Intake).  The Patient Access LPN is responsible for communicating with referral sources.  Collaborates with the Patient Access Manager, Clinical Managers, and Clinical Liaisons to meet the needs of referral sources and obtains the necessary clinical information to complete the referral process.

Responsibilities

 

Illustrative duties may include, but are not restricted to, one or more of the following:

  • Provides education and information regarding VNA programs, policies and services to the local community, healthcare facilities, physicians, patients and their families.
  • Works closely with skilled nursing facilities, physician’s offices, the community and VNA staff to facilitate the patient transition to home care.
  • Maintains and develops contacts and knowledge of the resources that are available within the local community.
  • Ability to use knowledge and skills to convert informational calls into home care referrals.
  • Ability and commitment to meet monthly budget numbers.
  • Responsible for the entry of patient information accurately and timely as part of the electronic medical records system.
  • Answers all incoming calls in a timely manner.  Effectively manages calls from external referral sources.  Maintains an accurate record of the origin and the response to all incoming calls.
  • Identifies unmet learning needs and seeks assistance from others in meeting those needs.
  • Complies with company policies and procedures.
  • Completes all required forms, reports, time cards, itineraries, etc, accurately and within designated timeframe.
  • Recognizes and follows established lines of communication.
  • Performs other duties as assigned.

Essential Functions:

This position would be responsible for providing review of patients’ home health care needs and coordination of referrals to home care and hospice

Responsibilities:

  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  • Coordinates referrals to home care to promote continuity of care
  • Provides individualized counseling to patient and family regarding available home care services
  • Establishes and maintains professional working relationship with hospital staff, physicians, and VNAHG staff
  • Ensures compliance with regulatory and statutory requirements
  • Provides back up coverage to assure availability of intake coordinator services
  • Adheres to the organization’s policy in regards to absenteeism and appearance

Qualifications

Qualifications and Requirements:

  • High School Diploma or GED.  Graduate of an accredited LPN program.  Associate's degree in Nursing preferred.
  • 1 year hospital experience.
  • Home care and/or Community Health experience is preferred.

·         Experience working as part of a team and working effectively with persons and communities from diverse cultural, social, and ethnic backgrounds.  The ability to work in a complex institutional structure requiring significant collaboration. 

·         Ability to work independently with minimal supervision.

·         Demonstrated ability to maintain strict confidentiality. 

 

Required Licenses, Registrations and/or Certifications:

  • Nursing Certification; licensed in the State of Ohio.
  • CPR certification is required.
  • Valid driver's license.
