Overview

Community Health Center (CHC) offers comprehensive, community based, preventive, primary care and wellness services for New Jersey residents of all ages at four convenient locations.CHC health care services are designed to meet the specific needs of the entire family – from annual check-ups and immunizations to sick visits, mental health counseling and critical prenatal care. 

 

Clinical Access Specialist is responsible for centralized Call Center clinical direction including: triage assessment, oversight of provider scheduling, and preventing/reducing gaps in care by assisting CHC patients over the phone.

Responsibilities

Principal Responsibilities:

  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationships with colleagues up, down, and across the organization
  • Coordinates centralized Call Center clinical direction including triage assessment based on information provided over the phone, review of medical history via EHR, and consultation with a provider as needed
  • Responsible for oversight of provider schedules to ensure adherence to scheduling guidelines, same day visit availability, balance of appointment types, and any other patient scheduling conventions
  • Liaisons with the clinical teams within the CHC sites to prioritize same day and/or urgent care
  • Maintains and updates patient records using the EHR
  • Functions as the EHR Superuser within the Call Center
  • Develops and enacts framework for Call Center escalation strategy in order to assist with appropriate routing of calls; educates the Patient Service Representatives in the same
  • Participates in activities that streamline business operations and institutes measures to improve customer service and patient care in conjunction with the Call Center Manager
  • Performs other duties as directed in accordance with the standard operating procedures

Qualifications

Job Competencies & Minimum Qualifications:

 

  • Proficient with Healthcare Management Systems, Microsoft Word, and Excel
  • RN; BSN preferred
  • Minimum of 3 years of clinical nursing experience required
  • 1+ year of ambulatory care experience at a Community Health Center preferred
  • Current CPR certification
  • Bilingual preferred – French Creole and/or Spanish
  • Car and valid NJ driver’s license
