The VNA Health Group is seeking a Baylor Registered Nurse  to join our dedicated Hospice Team in Monmouth County.

 

Baylor RNs to work two (2) twelve (12) hour weekend shifts per week at manager’s discretion but essentially the shifts are (Sat & Sun-  8:30am-8:30pm with 3- 15 min breaks) .  Baylor RNs shall be paid for thirty (30) hours for twenty four (24) hours worked and shall receive the same benefits as RNs, pro-rated to a thirty (30) hour workweek.  Paid the 30 hours but are excepted to get your paper work and follow-ups completed within those hours 24-30hrs.

Responsibilities

  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  • Assesses and evaluates the needs of patients with multiple disease processes and co morbidities and develops an appropriate plan of care and involves necessary disciplines
  • Documents all changes in patient’s condition and updates care plans and outcomes
  • Identifies teaching needs and plans appropriate strategies for individuals and families
  • Monitors quality and appropriateness of services provided
  • Provides and follows case management model of care
  • Adheres to agency infection control and safety policies

Qualifications

  • RN with Bachelor’s degree in Nursing preferred
  • 1+ year of supervised clinical experience as a registered nurse
  • Excellent verbal and written communication skills
  • Strong computer skills
  • Excellent organizational and interpersonal skills
  • Car and valid driver’s license
