Overview

VNA Health Group is the state’s largest not-for-profit community health provider of home health, hospice and community services. More than 1,500 certified home health aides, therapists, social workers, and visiting nurses bring their skilled care and compassion to people who need it most each day.

 

Visiting Nurse Association of Central Jersey Community Health Center, Inc. (VNACJ CHC), a Federally Qualified Health Center (FQHC), has provided comprehensive primary and preventive health care for infants, children, and adults of Monmouth County for more than 27 years. 

 

We are currently looking for a Advanced Practice Nurse or Physician Assistant, to join our team at our Primary Care Facility in Freehold, NJ. This position is responsible for providing and supervising the quality and quantity of health care services rendered to clients of all age groups at the Primary Care Center.

 

Responsibilities

Principal Responsibilities:

  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  • Provides primary care services to individual and family clients within established productivity guidelines
  • Provides client teaching to individuals and families when appropriate
  • Monitors quality and appropriateness of services provided
  • Identifies new services or programs relevant to agency’s mission and philosophy
  • Adheres to agency infection control and safety policies and guidelines of primary care regulatory agencies
  • Supervises staff in conjunction with the organization’s managerial and leadership practices

Qualifications

 

Job Competencies & Minimum Qualifications:

  • Licensure as an Advanced Practice Nurse in NJ//Physicians Assistant 
  • Master’s degree in Nursing from an NLN accredited program
  • Certified as a Family Nurse Practitioner with prescriptive authority; maintain a CDS and DEA
  • 2+ years of primary or ambulatory care nursing/Physicians Assistant experience required
  • Business or management degree or experience preferred with demonstrated leadership skills
  • Car and valid driver’s license

 

Working Conditions/Physical Demand: Working indoors.  Exposure to blood, body fluids and communicable diseases. Sitting, fine motor hand movement, standing and walking. Client/customer contact
