Position Summary:

 

We are currently looking for a Licensed Social Worker to join our team. The person in this role provides psychotherapeutic counseling and support to hospice patients and families, with a particular emphasis on those experiencing psychosocial and/or interpersonal conflict as well as to our pediatric hospice unit and children’s bereavement programming.

Responsibilities

Principal Responsibilities:

1. Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
2. Administers initial and ongoing psychosocial assessments and consultations of hospice patients and their families to determine their strengths, issues of concern, and treatment goals
3. Consults with interdisciplinary hospice team to discuss and evaluate the plan of care for each hospice patient and family, including review of clinical goals and treatment plans.
4. Advocates for hospice patients and their families to assist them in obtaining appropriate care and access to resources aimed at increasing their quality of life among interdisciplinary team, hospitals, long term care facilities, and community organizations.
5. Adheres to the organization’s policy and procedure on documentation
6. Participates in organizational wide QAPI program and attends agency sponsored in-service programs
7. Adheres to agency infection control and safety policies
8. Adheres to the organization’s policy in regards to absenteeism and appearance

Qualifications

Qualifications:

1. Master’s Degree in Social Work accredited by Council on Social Work Education required with current NJ LSW
2. 1+ year of supervised clinical experience in a healthcare setting; community health and/or pediatric healthcare experience preferred
3. Excellent verbal and written communication skills; creative arts experience preferred
4. Excellent interpersonal skills
5. Ability to work with multiethnic and diverse socioeconomic populations; ability to speak Spanish preferred

Working Conditions/Physical Demand: Services are provided in the patient’s home in a community or facility setting
