Overview

Community Health Center (CHC), VNA Health Group offers comprehensive, community based, preventive, primary care and wellness services for New Jersey residents of all ages at four convenient locations.CHC health care services are designed to meet the specific needs of the entire family – from annual check-ups and immunizations to sick visits, mental health counseling and critical prenatal care

 

We are seeking a Nurse Manager to support our Asbury Park, NJ facility.  Responsible for overseeing all aspects of clinical care provided within the designated site. Provides effective initial and ongoing orientation and training to all clinical employees. Partners with the site’s Front Office Manager to ensure that safe, high quality patient care is delivered following all established policies/procedures/workflows and that all Key Performance Indicator targets are met. Demonstrates a collaborative leadership style, building positive working relationships up, down, and across the organization.

Responsibilities

Principal Responsibilities:

  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with colleagues up, down, and across the organization
  • Assists site providers by directing and overseeing patient throughput
  • Coordinates access to clinical care for all patients, liaison between patient, care providers and team
  • Oversees case management for medically complex patients, advocating for patients with insurers and other outside resources
  • Performs related duties as directed in accordance with the standard operating procedures
  • Assists team in identifying patient care improvement needs and participates in quality initiatives
  • Maintains required logs and records in order to maintain regulatory compliance
  • Assists in coordination, management and documentation of medications, supplies, and referrals
  • Oversees the environment of care and adheres to agency infection control and safety policies and procedures; monitors site employee performance in the same
  • Participates in community outreach to promote CHC services and programs
  • Supervises RNs, LPNs, CMAs, and other clinical staff in conjunction with the organization’s managerial and leadership policies and procedures
  • Functions as the site’s Electronic Health Record Superuser

Qualifications

Job Competencies & Minimum Qualifications:

  • RN; BSN required
  • 5+ years of clinical nursing experience
  • 3+ year of ambulatory care experience, FQHC preferred
  • Current BLS certification
  • Venipuncture certification preferred
  • Car and valid NJ driver’s license
