Position Summary

Responsible for providing case management services to families of children and youth with Autism and  Special Healthcare Needs.  Position is hybrid with some travel to client’s home.

Responsibilities

  1. Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  2. Provides case management services to families including interviews to identify and assess their needs  and assists them with accessing federal, state and local services and support
  3. Submits data and reports as necessary to meet reporting requirements of the grant
  4. Participates in child case finding activities to identify gaps in service delivery
  5. Integrates new trends and developments in case management
  6. Adheres to agency infection control and safety policies
  7. Adheres to the organization’s policy in regards to absenteeism and appearance.

 

Qualifications

  1. BSW or MSW 
  2. Valid NJ SW certification/license (CSW/LSW/LCSW)
  3. 1+ years’ experience in pediatric case management for children and youth with special healthcare needs
  4. Strong computer skills
  5. Excellent verbal and written communication skills
  6. Strong organizational and interpersonal skills
  7. Valid NJ driver's license

 

 

Working Conditions/Physical Demand: Position is hybrid. Some work will be remote but visiting with clients in their homes or community as well as coming into the office is sometimes required.
