The Social Worker will be responsible for providing assessment of social and emotional problems that are, or are expected to be an impediment to the effective treatment of the patient’s medical condition or their rate of recovery.

Responsibilities

Here’s how you’ll move healthcare forward in this role:

  • Take part in a two-week clinical orientation to learn key procedures, the EMR system, compassionate care, and safety
  • Develops and implements a social work plan of care
  • Collaborates with professional staff on psychosocial problems and appropriate interventions
  • Provides patients and families with information on community resource options and assists in linking with needed resources
  • Adheres to the organization’s policy and procedure on documentation
  • Adheres to agency infection control and safety policies

Qualifications

The ideal qualifications for the Social Worker, include:

  • Masters Degree in Social Work required with current NJ LSW
  • 1+ year of supervised clinical experience; community health experience preferred
  • Excellent verbal and written communication skills
  • Excellent interpersonal skills
  • Ability to work with multiethnic and diverse socioeconomic populations
  • Spanish speaking language a plus

 Working Conditions/Physical Demand: Services are provided in the patient’s home in a community or facility setting
