Overview

Community Health Center (CHC) offers comprehensive, community based, preventive, primary care and wellness services for New Jersey residents of all ages at four convenient locations.CHC health care services are designed to meet the specific needs of the entire family – from annual check-ups and immunizations to sick visits, mental health counseling and critical prenatal care. 

 

The Community Health Center (CHC) is currently seeking a Recovery Care Program Assistant.  This position will be responsible for providing assistance to patients with substance use disorder in activities related to their health care such as navigating the health care and housing, coordinating delivery of injectable medications, and providing as needed administrative support to the entire Medication-Assisted Treatment team.

Responsibilities

 

  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers;
  • Builds effective, working relationships with patients, healthcare team and resource providers to improve coordination of patient care and reduce barriers;
  • Evaluate barriers to care and treatment, develop an action plan with the patient to overcome barriers, and support patient with overcoming their barriers;
  • Conducts initial telephone intakes with new patients to gauge the appropriate level of treatment and refer to the applicable MAT program for care;
  • Support Peer Navigator with ongoing tasks as needed including routine check-in calls and conducting additional follow-ups and outreach as needed;
  • Oversee the scheduling, delivery, and receipt of injectable medications in advance of patient appointments;
  • Serve as a part of the VNAHG’s CFHI’s Prevention Resource Network and VNACJ CHC interdisciplinary teams, functioning as a team member;
  • Evaluate the patients’ health care literacy, assist them when barriers are identified, and make referrals and schedule appointments as needed;
  • Works with the Peer Navigator and MAT team to connect patients to relevant community resources with the goal of enhancing the patient's health while reducing emergency room utilization and hospital admissions
  • Collaborates with Patient Care Coordinator to facilitate specialty appointments, and obtain prior authorizations to assure MAT patients are able to access appropriate medications, equipment and service to ensure that the MAT patients specific healthcare needs are met;
  • Participates in community outreach events to raise awareness of VNACJ CHC services;
  • Updates and maintains accurate client data in compliance with privacy and confidentiality standards;
  • Adheres to the organization’s policy in regards to absenteeism and appearance.

Qualifications

 

  • High School diploma/GED, Associates or Bachelor Degree in health education or social work preferred
  • 1-2 years work related experience with outreach experience
  • Experience working with individuals with substance abuse disorder
  • Strong computer skills
  • Excellent communication, customer service and interpersonal skills
  • Current and valid driver’s license, auto insurance and registered vehicle

Working Conditions/Physical Demand: Business Office Environment within a fast-paced healthcare center with heavy customer service, phones and computer use. Requires travel to branch locations.
