Overview

Responsible for providing peer support to patients with substance use disorder including assisting with referrals to recovery resources, conducting wellness visits, and providing direct support to clients at mobile harm reduction sites.

Responsibilities

  1. Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
  2. Participate in weekly community outreach activities on a Mobile Harm Reduction Unit and at the VNA Harm Reduction Center.
  3. Maintain a caseload of clients to ensure retention in care is preserved through routine check-in calls, visit reminders, and additional follow-ups and outreach as needed.
  4. Serve as a part of the VNAHG’s CFHI’s Prevention Resource Network and VNACJ CHC interdisciplinary teams, functioning as a team member.
  5. Responsible to collaborate on GPRA documentation and support administration in collecting data related to patient population.
  6. Work with the case manager to evaluate the clients’ health care, social service, and support needs, assist them when barriers are identified, and make referrals and schedule appointments as needed.
  7. Conducts direct community outreach with existing clients, including performing home/wellness checks and accompanying clients to outside appointments for healthcare, social services, etc.
  8. Participates in community outreach events to raise awareness of VNA services.
  9. Updates and maintains accurate client data in compliance with privacy and confidentiality standards.
  10. Adheres to the organization’s policy in regards to absenteeism and appearance.
  11. Maintain, organize, and keep sanitized the mobile harm reduction van.

 

Qualifications

  1. High School diploma/GED, Associates or Bachelor Degree in health education or social work preferred
  2. 1-2 years work related experience with outreach experience
  3. Experience working with individuals with substance abuse disorder
  4. Strong computer skills
  5. Excellent communication, customer service and interpersonal skills
  6. Current and valid driver’s license, auto insurance and registered vehicle
