Overview

Monmouth ACTS Community Coordinator will assist with in person client intakes, connecting clients to community resources, community outreach to residents, connecting with new community resources and service providers.

Responsibilities

 

  • Assess the service needs of residents and linking them to the appropriate resources and providers
  • Assist residents with applications or forms and scheduling appointments if necessary
  • Assist residents in responding to any issues that occur during the delivery of services Communicate directly with the service organizations when necessary to facilitate communication and explore solutions with resident voice at the center
  • Regularly follow up with the resident to ensure the services being provided meet their needs, addressing any challenges or service disruptions
  • Follow up with residents regularly to assist and ensure their satisfaction. Report any system challenges, gaps or areas for improvement

Qualifications

  • Education: High school diploma, college degree preferred
  • 2+years of work experience, ideally involving community. Individuals and organizations
  • Strong computer skills including mobile device, Apps and databases
  • Car, driver’s license and insurance required
  • Excellent verbal and written communication and interpersonal skills
  • Strong organizational and data collection skills
  • Bilingual preferred
  • Behavioral Health Experience Preferred
