Responsibilities

How You’ll Make a Difference: 

 Participate in a two week clinical orientation 

 Assess, evaluate, and develop care plans for patients 

 Provide education and care strategies for patients and families 

 Collaborate with VNA clinicians and external providers 

 Document changes and update care plans

Qualifications

 Active RN License (BSN preferred) 

 1+ year of supervised RN experience (home health or medsurg preferred) 

 Excellent communication and problem solving skills 

 Car and valid driver’s license

 

Working Conditions/Physical Demand: Working indoors and outdoors with clinical equipment and use of phone and computer; Job characteristics: Client and Customer contact
