Your Role:

• Provide in-home physical therapy evaluations and treatments.
• Develop personalized care plans in collaboration with physicians.
• Document progress and update care plans as needed.
• Educate patients and families for better outcomes.
• Ensure high standards of care and adhere to safety protocols.

What You Bring:

• Valid Physical Therapy License in NJ.
• 2-3 years of clinical experience (community-based experience preferred).
• Strong communication and computer skills.
• A valid NJ driver’s license.

Working Conditions/Physical Demand: Working indoors/outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, use of phone and tablet for documentation
