We are currently looking for a Registered Nurse to join our team as a Home Care Intake Coordinator on a per diem basis. The position would be responsible for providing assessment of patients’ home health care needs and coordination of referrals to home care and hospice. The person in this role will be stationed at Jersey City Medical Center.
Working Conditions/Physical Demand: Fast paced health care environment with phone, scanner, email, fine motor hand movement and computer use. Use of personal automobile with valid insurance and driver’s license
