Overview

Responsible for providing assessment and referrals for individuals living with HIV to increase access to medical care and other needed resources.

 

Responsibilities

  1. Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  2. Performs assessment of individual needs and develops and documents appropriate plan of care
  3. Anticipates insurance lapses, assists with required insurance and medication coverage applications
  4. Assesses for related program eligibility, mental health, housing services, substance use services, and other entitlements
  5. Documents all required data into the reporting systems including CareWare.  Provides reports as requested by Program Manager
  6. Provides ongoing follow up and evaluation of the effectiveness of services being provided
  7. Adheres to the procedures and documentation as required by Part B Standards of Care for Case Management
  8. Adheres to agency infection control and safety policies
  9. Adheres to the organization’s policy in regards to absenteeism and appearance

 

 

 

Qualifications

 

  1. Bachelor’s degree in Social Sciences or related field
  2. 1+ years of HIV/AIDS work related experience
  3. Knowledge of cultural and lifestyle diversity issues
  4. Strong organizational and interpersonal skills
  5. Bi-Lingual Spanish preferred
