The VNA Health Group is seeking a full time RN to join our dedicated Hospice Team in Ocean County.

Responsibilities

  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  • Assesses and evaluates the needs of patients with life threatening and life limiting illnesses and their families and develops an appropriate plan of care
  • Identifies teaching needs and plans appropriate strategies for individuals and families
  • Monitors quality and appropriateness of services provided
  • Collaborates with patient and family, other members of the disciplinary team and other health care providers in providing patient and family care
  • Documents all changes in patient’s condition and updates care plans and outcomes
  • Adheres to agency infection control and safety policies
  • Adheres to the organization’s policy in regards to absenteeism and appearance
  • Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position

Qualifications

  • RN with Bachelor’s degree in Nursing
  • Certification in Hospice Palliative Care Nurse and Critical Care and Critical Care skills preferred
  • 1+ year of supervised clinical experience as a registered nurse
  • Excellent communication skills
  • Strong Computer skills
  • Excellent organizational and interpersonal skills

Working Conditions/Physical Demand: Working indoors/outdoors with clinical equipment and use of phone and computer; car and valid NJ Driver’s license
