We are currently looking for a Hospice HHA Coordinator to join our team at our office in West Orange, NJ. The person in this role is responsible for coordinating interdisciplinary team meetings. 

Responsibilities

1. Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
2. Prepares and coordinates team meetings
3. Reviews records to ensure COP compliance for Hospice
4. Performs data collection of open notes and reports to Hospice Managers
5. Responsible for tracking recertifications due and preparation of Recertification packets for RN’s .
6. Answers staff related calls and refers clinical questions to appropriate clinical team member
7. Prepares and submits monthly Level of Care Report for appropriate Teams to Director of Hospice Billing & Compliance monthly.
8. Adheres to the organization’s policy in regards to absenteeism and appearance

 

Qualifications

1. Bachelor’s degree in health related field preferred
2. Excellent verbal and written communication skills
3. Strong computer skills
4. Strong organizational skills
5. Strong customer service and interpersonal skills
