Program Manager - Integrated Behavioral Health

 

Community Health Center (CHC) offers comprehensive, community based, preventive, primary care and wellness services for New Jersey residents of all ages at four convenient locations.CHC health care services are designed to meet the specific needs of the entire family – from annual check-ups and immunizations to sick visits, mental health counseling and critical prenatal care. 

 

The program manager is responsible for managing the integrated behavioral health programs in one or more VNACJ CHC health center locations and for ensuring all grant deliverables are met, monitoring program performance, and providing support to patients, community members, and program staff.

Responsibilities

  •  Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationships with co-workers up, down, and across the organization
  •  Ensures all grant objectives are met and tracks program deliverables, procurement, and expenditures as needed; monitors program growth and performance and responds to both positive and negative trends
  •  Builds effective working relationships with patients, health care team and resource providers (internal and external) to improve coordination of patient care and reduce barriers to access
  • Provides clinical administrative support including submitting prior authorizations
  •  Oversees the scheduling, delivery, and receipt of injectable medications in advance of patient appointments
  •  Supervises the unlicensed support staff associated with the program, including but not limited to, Peer Navigator, Behavioral Health Assistant, and/or Program Assistant
  • Ensures that the integrated behavioral health programs are not segregated from primary care and other programs and service lines to best meet the needs of health center patients and staff
  •  Conducts outreach to ensure partner organizations and community members are aware of the integrated behavioral health programs at VNACJ CHC, including tabling at events (both weekday and weekend); partners with Community Outreach team in the same
  •  Evaluate the program’s efficacy in addressing patient needs, including meeting UDS measures
  •  Participates in Quality and Risk Management program and other applicable committees
  •  Updates and maintains accurate patient data in compliance with privacy and confidentiality standards
  •  Adheres to the organization’s policy in regards to absenteeism and appearance and manages others in the same

 

 

Qualifications

  •  High School diploma/GED required; Associates or Bachelor Degree in public health, health education or social work preferred
  •  2-3 years related work experience in a community based program or healthcare setting required
  •  Experience working with individuals with mental health concerns including substance abuse disorder
  •  Strong computer skills
  •  Excellent communication, customer service, and interpersonal skills
  •  Current and valid driver’s license, auto insurance, and registered vehicle

 

Working Conditions/Physical Demand: Business Office Environment within a fast-paced healthcare center with heavy customer service, phones and computer use. Requires travel to branch locations as needed.
