The VNA Health Group is looking for a Full Time Physical Therapist to join our team in Middlesex County. The position would be responsible for  providing Physical Therapy evaluation and treatment to patients in their homes or alternate community settings

The Physical Therapist will be responsible for  providing physical therapy evaluation and treatment to patients in their homes or alternate community settings.

Responsibilities

Here’s how you’ll move healthcare forward in this role:

  • Take part in a two-week clinical orientation to learn key procedures, the EMR system, compassionate care, and safety.
  • Assesses and evaluates client for physical therapy needs and develops and implements appropriate plan of action in collaboration with the physician.
  • Documents all changes in client’s condition and updates care plans and outcomes accordingly.
  • Identifies counseling needs and plans appropriate teaching strategies.
  • Monitors quality and appropriateness of services provided.

Qualifications

The ideal qualifications for the Physical Therapist, Homecare, include:

  • Physical Therapy Licensure in NJ required
  • 2-3 years of prior clinical experience and community based experience preferred
  • Strong computer skills
  • Strong communication and interpersonal skills
  • Valid NJ drivers license 

Working Conditions/Physical Demand: Working indoors/outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, use of phone and tablet for documentation
