Family Nurse Practitioner, Primary Care

 

Community Health Center (CHC) offers comprehensive, community based, preventive, primary care and wellness services for New Jersey residents of all ages at four convenient locations.CHC health care services are designed to meet the specific needs of the entire family – from annual check-ups and immunizations to sick visits, mental health counseling and critical prenatal care. 

 

We are currently looking for a passionate Advanced Practice Nurse, to join our Primary Care Center team. In this full-time role, you would be responsible for providing holistic primary care services in collaboration with other Community Health Center of Asbury Park (CHC) staff to patients of all age groups. Responsibilities

  1. Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  2. Provides direct family and patient centered services to primary care patients at the sites within established productivity guidelines
  3. Maintains complete patient records using the Electronic Medical record and complies with all reporting mechanisms
  4. Ensures compliance with the New Jersey Department of Health Ambulatory Care Regulations for Licensure as well as requirements of other licensing and accrediting bodies, health maintenance organizations and other insurance carriers
  5. Collaborates in team based care in the Patient Centered Medical Home model
  6. Participates in Peer Review and other Quality Improvement activities
  7. Collaborates in the provision of in-service education for staff
  8. Adheres to CHC infection control and safety policies, and guidelines of regulatory agencies
  9. Participates in community outreach programs regarding CHC services and programs
  10. Supervises staff in conjunction with the organization’s managerial and leadership practices
  11. Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position

Qualifications

  1. Licensure as an Advanced Practice Nurse in NJ
  2. Master’s degree in Nursing from an NLN accredited program
  3. Certified as a Family Nurse Practitioner with prescriptive authority; maintain a CDS and DEA
  4. 2+ years of primary or ambulatory care advanced practice nursing experience preferred; pediatric experience preferred.
  5. Car and valid driver’s license

 

Working Conditions/Physical Demand: Working indoors. Exposure to blood, body fluids and communicable diseases. Sitting, fine motor hand movement, standing and walking. Client/customer contact
