We are currently looking for a Nurse Practitioner to join our team on a full time basis to cover sections of Ocean County, NJ area. Responsible for ensuring delivery of high quality care practices in the treatment of symptoms and promoting the highest quality of life for homebound patients and their families

1. Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
2. Assesses, diagnoses and develops appropriate plan of care to treat homebound patients
3. Monitors the patient’s condition and responsiveness to the treatment provided
4. Acts as an agency representative with patients and their families as well as with other health agencies, groups and organizations
5. Participates in organizational wide QAPI program and attends agency sponsored in-service programs
6. Adheres to agency infection control and safety policies
7. Adheres to the organization’s policy in regards to absenteeism and appearance
1. Master’s degree in Nursing required from an NLN accredited program
2. Licensure as an Advanced Practice Nurse (APN) in NJ with prescriptive authority; maintain a CDS and DEA
3. National certification as an Adult or Geriatric Nurse Practitioner
4. 2 + years of APN experience and/or significant experience as a registered nurse
5. Excellent communication, organizational and interpersonal skills
6. Strong computer skills
Working Conditions/Physical Demand: Working indoors/outdoors with Client/Customer contact and clinical equipment and use of phone and computer; valid driver’s license and automobile insurance
