A proud past. A compelling future. A future that includes you.

VNA Health Group is looking for a Per Diem Hospice Chaplain to join our team — someone with a heart for healing, a listening ear, and a spiritual presence that brings peace and comfort during life’s most meaningful transitions. This position will support patients and families across Monmouth, Middlesex, and Ocean Counties.

Responsibilities

What You'll Do:

  • Provide compassionate spiritual counsel and emotional support to hospice patients and their families — both during care and in bereavement.

  • Serve as a vital member of the Interdisciplinary Care Team, helping assess spiritual needs and contributing to the care plan.

  • Offer rituals, prayer, and sacramental ministry as desired by patients and families, in a non-denominational, interfaith environment.

  • Coordinate with community clergy and religious leaders to connect families with their faith traditions.

  • Lead memorial or funeral services for families not affiliated with a church or spiritual leader.

  • Participate in bereavement care and community outreach/education around hospice and spiritual care.

  • Document care in accordance with Medicare and CHAP guidelines using our advanced digital tools.

Qualifications

What you Bring:

  • Bachelor’s degree required; Master’s in Theology/Divinity or equivalent strongly preferred

  • Ecclesiastical endorsement and good standing in a religious body (preferred)

  • At least 3 years in active pastoral care ministry and 2 units of CPE required

  • Experience supporting individuals through end-of-life transitions and grief

  • Ability to work effectively within interfaith, non-denominational settings

  • Strong interpersonal, counseling, and communication skills

  • Valid NJ driver’s license and auto insurance

Working Conditions/Physical Demand: Services are provided in the patient’s home in a community or facility setting.
