The Children & Family Health Institute (CFHI) at VNA Health Group is seeking a dedicated Home Visitor RN to deliver essential care and support through the Nurse-Family Partnership (NFP) program in Ocean County, New Jersey. This role offers a unique opportunity to guide families toward healthier pregnancies, positive child development, and brighter futures.

Responsibilities

As a Home Visitor RN, you will:

    • Conduct in-home visits to provide nursing services aligned with the NFP model and Visit-to-Visit guidelines, managing a caseload of up to 25 clients.
    • Assess and address the health and developmental needs of women and their families.
    • Offer counseling and instruction to help families achieve targeted health and life goals.
    • Build and maintain strong community relationships to facilitate client referrals and resource connections.
    • Attend weekly team meetings, case conferences, and clinical supervision sessions.
    • Mentor and precept new hires and support student nurses during home visits as needed.
    • Ensure compliance with agency infection control and safety policies.
    • Maintain professionalism in adherence to organizational policies on absenteeism and appearance.

Qualifications

Qualifications

  • Bachelor’s degree in Nursing required.
  • 2-3 years of relevant nursing experience; home visiting or maternal/child health experience preferred.
  • Proficiency in computer use, excellent verbal and written communication, strong organizational and interpersonal abilities.
  • Bilingual (Spanish-speaking) candidates are a plus.
  • A commitment to CFHI’s mission and the ability to build trusting relationships with clients and colleagues.
