As a Registered Nurse Home Health for Middlesex County at VNA Health Group, you’ll be an integral part of a team dedicated to providing high-quality, compassionate care to patients who need it most. You’ll make a direct impact on their lives by assessing their health needs, developing personalized care plans, and empowering patients and families with the knowledge they need to live healthier lives. How You’ll Make a Difference:
Participate in a two week clinical orientation
Assess, evaluate, and develop care plans for patients
Provide education and care strategies for patients and families
Collaborate with VNA clinicians and external providers
Document changes and update care plans
Active RN License (BSN preferred)
1+ year of supervised RN experience (home health or medsurg preferred)
Excellent communication and problem solving skills
Car and valid driver’s license
Working Conditions/Physical Demand: Working indoors and outdoors with clinical equipment and use of phone and computer; Job characteristics: Client and Customer contact
