As a Clinical Data Coordinator & OASIS Reviewer, you’ll play a key role in supporting our commitment to high-quality home health care.

 

HCS-O Certification Required

Responsibilities

What You’ll Do:

 

Review OASIS documentation for admission, resumption, recertification, and discharge assessments for Medicare, Medicaid, and HMO cases.

Ensure all documentation aligns with the Plan of Care and medication profile.

Collaborate with field clinicians to validate documentation accuracy and completeness.

Qualifications

  • RN with Bachelor’s degree in Nursing  
  • OASIS Certification required 
  • 3-5 years of home care experience
  • Strong knowledge of OASIS and Medicare/Medicaid guidelines
  • Strong computer skills  
  • Excellent verbal and written communication skills
  • Excellent interpersonal skills

Working Conditions/Physical Demand: Business Office Environment with phone and computer use

 

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $70,200 to USD $159,171.19. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
