We are currently looking to hire for our Hospice Administrative Coordinator, based at our office in West Orange, NJ. The person in this role is responsible for providing patient counseling and administrative coordination of all hospice referrals under the direction of a Registered Nurse.

Responsibilities

  • Supports organization's mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  • Coordinates hospice referrals to promote continuity of care
  • Provides individualized counseling to patient and family regarding available hospice services
  • Establishes and maintains professional working relationship with all stakeholders including but not limited to hospital staff, physicians, referral sources and VNAHG staff
  • Ensures compliance with regulatory and statutory requirements
  • Works in conjunction with RN regarding complex referrals
  • Works under direct supervision of RN for completion of MD orders
  • There may be assignment of projects and/or tasks assigned as needed to support the Hospice business.
  • Positon requires working in office. This is not a remote position.

Qualifications

  • Bachelor’s Degree in Healthcare Administration or related field
  • 2+ years of hospice experience
  • Excellent verbal and written communication skills
  • Excellent interpersonal and organizational skills
  • Strong client focus, articulate and creative in problem solving
  • Strong decision making skills
  • Competency with technology skills including computer and system use; HCHB, MS Outlook, Word, Excel and PowerPoint

 

Working Conditions/Physical Demand: Fast paced health care environment with phone, scanner, email, fine motor hand movement and computer use. Able to multi task. Use of personal automobile with valid insurance and driver's license. Minimal lifting in office setting of 5 to 10lbs.

 

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $33,280- USD 87,500. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
