Overview

Licensed Practical Nurse (LPN) – Homecare
Location: Essex County, NJ
 
Position Summary:
VNA Health Group is seeking a compassionate Licensed Practical Nurse (LPN) to join our homecare team. In this role, you will provide nursing services and quality care to patients in their homes under the direction of a Registered Nurse. Territory:
This role is community-specific to Livingston, The Caldwells, and surrounding areas within Essex County, NJ. Nurses scheduled in this area will remain within these communities, allowing for consistent scheduling and a more efficient workday.

Responsibilities

What You Will Do:

  • Provide nursing care as directed by the Registered Nurse and the plan of care
  • Assist patients in following preventative and rehabilitative nursing procedures
  • Monitor quality and appropriateness of services provided
  • Document changes in condition and update care plans and outcomes accordingly
  • Establish and maintain professional working relationships with healthcare providers and team members
  • Comply with infection control, safety, and organizational policies

Qualifications

What You Will Need:

  • Graduate of an NLN-accredited LPN program
  • Current NJ LPN license
  • 2+ years of med/surg or acute care experience
  • 1-2 years of homecare experience preferred
  • Strong communication, computer, and organizational skills
  • Valid NJ driver’s license and reliable transportation
  • Willingness to travel within assigned Essex County territory

Working Conditions/Physical Demand:
Work includes both indoor and outdoor settings. Requires use of clinical equipment, phone, and computer; frequent travel to patient homes.

 

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $53,040 to USD $95,662.50. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
