As a Registered Nurse Home Health for Mercer County at VNA Health Group, you’ll be an integral part of a team dedicated to providing high-quality, compassionate care to patients who need it most. You’ll make a direct impact on their lives by assessing their health needs, developing personalized care plans, and empowering patients and families with the knowledge they need to live healthier lives. • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
• Assesses and evaluates the needs of patients with multiple disease processes and co morbidities and develops an appropriate plan of care
• Documents all changes in patient’s condition and updates care plans and outcomes
• Identifies teaching needs and plans appropriate strategies for individuals and families
• Monitors quality and appropriateness of services provided
• Establishes and maintains professional working relationship with health care providers and other members of the disciplinary teams in providing patient and family care
• Adheres to agency infection control and safety policies
• Adheres to the organization’s policy in regards to absenteeism and appearance
• Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position

• RN with Bachelor’s degree in Nursing preferred
• 1+ year of supervised clinical experience as a registered nurse
• Excellent verbal and written communication skills
• Strong computer skills
• Excellent organizational and interpersonal skills
• Car and valid driver’s license

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $70,200 to USD $159,171.19. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
