Children and Family Health Institute at Visiting Nurse Association Health Group supports people of all ages with more than 20 programs and services. Community Health Worker InCK will help families in Monmouth/Ocean County identify strengths and challenges and carry out the Goals of the Integrated Care Plan(ICP). Drawing on a strong network of NJ InCK partners and using new technology for communicating and making referrals, they will ensure the children/youth are connected to appropriate Core Child Services including physical and behavioral health care, child care and preschool, school delivered services, maternal child health programs, food and housing supports, legal services, and the child welfare system as needed. Community Health Workers are intended to be familiar with the geographic area to which they are assigned and to have cultural affiliations and in some cases language capacities that allow them to best serve the families and youth with whom they work.

Working Conditions/Physical Demand: Working indoors at VNA locations and with travel to community locations to meet families/youth and providers of Core Child Services. Occasional participation in community events that may be held out of doors.
In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $39,780.00 to USD $100,741.875. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
