Position Summary:

As a MOUD RN, you will provide culturally responsive, evidence-based nursing care to individuals impacted by opioid use, both in the PRN office in Monmouth County, and in community settings. You will coordinate access to Medication for Opioid Use Disorder (MOUD) through hybrid telehealth appointments in collaboration with licensed providers. This role also involves helping clients overcome barriers to accessing medication as they arise, while integrating harm reduction practices into every aspect of care.

Responsibilities

  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationships with co-workers
  • Work with harm reduction staff to identify participants interested and eligible for MOUD treatment
  • Collaborate with existing MOUD providers to connect participants into care via weekly hybrid telehealth appointments in the community and at designated Harm Reduction sites
  • Conduct oral drug screens and/or urine drug screens and report findings to licensed MOUD provider
  • Conduct basic vital signs and health assessments and report findings to the licensed MOUD provider
  • Provide direct assistance with accessing prescription MOUD including troubleshooting barriers with pharmacies and insurance as they arise
  • Responsible for collaborating on GPRA documentation and supporting administration in collecting data related to patient population
  • Provide referral and linkage to primary care and support services
  • Conducts direct community outreach with existing clients, including performing home/wellness checks and accompanying clients to outside appointments for healthcare, social services, etc
  • Provide safer injection education and overdose prevention education
  • Provide Naloxone distribution and training
  • Report all data on the designated state reporting systems
  • Provide monthly reports as required

Qualifications

  • Licensure Registered Nurse in New Jersey
  • Must have experience working with hard-to-reach populations such as homeless, injection drug users, and clients infected or affected by HIV/AIDS
  • Minimum 2 years community nursing experience
  • Proficient in computer skills, including use of Word, Excel, and PowerPoint
  • Current Adult CPR Certification
  • Valid NJ drivers license required and auto insurance
  • Bilingual candidates(Spanish-speaking) preferred

Working Conditions/Physical Demand: Working indoors and outdoors with client contact

 

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $70,200.00 to USD $159,171.19. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
