We are currently looking for an Physical Therapist to join our team and become a Rehab Supervisor who will be responsible for planning and directing the rehabilitation services in a homecare setting for the assigned region. Responsible for providing occupational therapy evaluation and treatment to patients in their homes or alternate community settings

Responsibilities

1. Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
2. Assesses and evaluates client for occupational therapy needs and develops and implements appropriate plan of action in collaboration with the physician.
3. Documents all changes in client’s condition and updates care plans and outcomes accordingly.
4. Identifies patient, caregiver, and family counseling and education needs and plans appropriate teaching strategies.
5. Monitors quality and appropriateness of services provided.
6. Assists in managing and supervising the PTAs on staff and the overall program.
7. Participates in VNAHG’s QAPI and in-service programs.
8. Adheres to and ensures compliance agency infection control and safety policies.
9. Adheres to the organization’s policy in regards to absenteeism and appearance.
10. Plans, develops and manages the rehab program and facilitates the appropriate utilization of rehab services to assure positive patient outcomes.
11. Provides evaluative home visits and develops and manages plan of care for the staff.
12. Develops, implements and monitors quality outcomes for rehab services.

Qualifications

1. Physical Therapy Licensure in NJ
2. 2-3 years of prior clinical experience and community based experience preferred
3. Strong computer skills
4. Strong communication and interpersonal skills
5. Valid NJ Driver’s license

 

Working Conditions/Physical Demand: Working indoors/outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, use of phone, computer and tablet for documentation.

 

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $72,885.38 to USD $179,750.00. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
