Clinical Manager – Access Center
Schedule: Full Time

 

Position Summary:
The VNA Health Group is seeking an experienced Clinical Manager to provide leadership and clinical oversight to our Access Center team. This role is integral to ensuring the efficient coordination and quality of services provided through our centralized intake and referral operations. The Clinical Manager will guide a team of professionals, supporting patient access to home health and hospice services while upholding the organization’s mission of delivering high-quality, patient- and family-centered care.

Responsibilities

 

What You Will Do:

  • Foster a culture of excellence and teamwork in alignment with VNA’s mission and values

  • Provide clinical oversight and support for Access Center operations

  • Review and document clinical communication for all referrals received

  • Supervise and mentor Access Center nurses and support staff

  • Serve as a clinical resource for intake-related decision-making and eligibility assessments

  • Collaborate with physicians, referral sources, and internal teams to ensure smooth patient onboarding

  • Contribute to continuous process improvement and service optimization

  • Ensure compliance with agency policies and regulatory standards

Qualifications

What You Will Need:

  • Active New Jersey RN license required

  • Bachelor’s degree in Nursing preferred

  • Minimum 2–3 years of experience in community health, home care, or hospice nursing

  • Previous supervisory or clinical leadership experience preferred

  • Strong clinical judgment and attention to detail

  • Excellent communication, organizational, and interpersonal skills

  • Proficiency in Microsoft Office and electronic health record systems

  • Ability to work in a fast-paced, collaborative environment

Working Conditions/Physical Demand: Business Office Environment within a fast paced call center with heavy phone and computer use

 

 

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $70,200.00 to USD $159,171.19. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
