Registered Nurse – Hospice Liaison
Location: Middlesex County, NJ
Schedule: Full Time

 

Position Summary:
VNA Health Group is seeking a dedicated Hospice Liaison RN to assess patients for hospice eligibility, coordinate admissions, and serve as a clinical resource to referral sources in Middlesex County. This role fosters continuity of care while promoting awareness and access to hospice services.

Responsibilities

What You Will Do:

  • Assess hospice eligibility and coordinate admissions

  • Support patients and families during transitions of care

  • Serve as a liaison to hospitals, physicians, and referral partners

  • Promote hospice services through education and outreach

  • Maintain accurate documentation and ensure regulatory compliance

 

 

Qualifications

What You Will Need:

  • NJ RN license; BSN preferred

  • Hospice or home care experience strongly preferred

  • Excellent communication and clinical assessment skills

  • Proficient in Microsoft Office and EMR systems

  • Valid NJ driver’s license, reliable vehicle, and insurance

Working Conditions/Physical Demand: Fast paced health care environment with phone, scanner, email, fine motor hand movement and computer use. Use of personal automobile with valid insurance and driver’s license

 

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $70,200.00 to USD $159,171.19. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
