Overview

Registered Nurse – Manager of Quality & Performance Improvement

VNA Health Group, a CHAP-accredited leader in home health and hospice, is seeking an experienced Registered Nurse to support quality and performance improvement initiatives. This role plays a vital part in promoting clinical excellence, regulatory compliance, and ongoing improvement across our programs.

Responsibilities

What You’ll Do:

  • Oversee and manage the Quality Assurance & Performance Improvement (QAPI) program

  • Ensure compliance with CHAP standards, CMS regulations, and Conditions of Participation

  • Analyze clinical data, identify trends, and lead performance improvement initiatives

  • Conduct audits, chart reviews, and risk assessments

  • Lead efforts related to patient safety, satisfaction, and STAR ratings

  • Prepare for and support CHAP accreditation surveys and internal audits

  • Educate staff on quality standards, documentation practices, and clinical excellence

  • Collaborate with leadership and interdisciplinary teams to drive outcomes

Qualifications

  • RN with Bachelor’s in Nursing or related field (Master’s preferred)

  • 5+ years of nursing experience, including 2+ in quality/performance improvement

  • OASIS certification and PI tools knowledge preferred

  • Strong communication, leadership, and presentation skills

  • Proficient in medical terminology, documentation, and computer systems

  • Excellent organizational and interpersonal skills

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $72,885.38 to USD $179,750. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
