VNA Health Group, a CHAP-accredited leader in home health and hospice, is seeking an experienced Registered Nurse to support quality and performance improvement initiatives. This role plays a vital part in promoting clinical excellence, regulatory compliance, and ongoing improvement across our programs.
Oversee and manage the Quality Assurance & Performance Improvement (QAPI) program
Ensure compliance with CHAP standards, CMS regulations, and Conditions of Participation
Analyze clinical data, identify trends, and lead performance improvement initiatives
Conduct audits, chart reviews, and risk assessments
Lead efforts related to patient safety, satisfaction, and STAR ratings
Prepare for and support CHAP accreditation surveys and internal audits
Educate staff on quality standards, documentation practices, and clinical excellence
Collaborate with leadership and interdisciplinary teams to drive outcomes
RN with Bachelor’s in Nursing or related field (Master’s preferred)
5+ years of nursing experience, including 2+ in quality/performance improvement
OASIS certification and PI tools knowledge preferred
Strong communication, leadership, and presentation skills
Proficient in medical terminology, documentation, and computer systems
Excellent organizational and interpersonal skills

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $72,885.38 to USD $179,750.
