We are currently looking for a Front Desk Coordinator to join our team at the Keyport Facility. The position would be responsible for providing coordination of the front office operations at the Community Health Center.
Responsible for providing peer support to patients with substance use disorder including assisting with referrals to recovery resources, conducting wellness visits, and providing direct support to clients at mobile harm reduction sites.
Responsible for providing comprehensive nursing services to women and their families eligible for the Nurse Family Partnership program

We are currently looking for a Registered Nurse to join our team as a Home Care Intake Coordinator. The position would be responsible for providing assessment of patients’ home health care needs and coordination of referrals to home care and hospice. The person in this role will be stationed at Newark Beth Israel Medical Center or one of the other major hospitals we partner with in Essex & Hudson County.
This clinical leadership position provides day-to-day management and oversight of patient throughput through our flagship health center location in Asbury Park, including primary care and two critical, specialized programs within VNACJ Community Health Center, a federally qualified health center serving Monmouth County, NJ.
The two specialized programs under the oversight of this leader are: LGBTQ Center for Health and Wellness and Ryan White Part C, serving patients in the eastern (Asbury Park) and western (Freehold) part of Monmouth County.
The LGBTQ Center for Health and Wellness provides inclusive, patient-centered primary health care and specialty services to a vibrant, but also underserved community.
The Ryan White Part C Program provides comprehensive HIV primary care.
Responsible for providing leadership and oversight of large community vaccination clinics throughout New Jersey. This role’s main objective is to build relationships in priority communities, community-based organizations and health care providers to coordinate mobile vaccination clinics (COVID-19 and flu or otherwise designated by the State of NJ). The Mobile Clinic Manager is responsible for reviewing the data collected at the mobile vaccination sites to ensure accurate reporting for both internal and external use.

We are currently looking for an hourly per diem Registered Dietitian Nutritionist for Ocean & Burlington County to join our team. This position would be responsible for providing nutrition assessment and counseling to home care patients and their families.
