We are currently looking for a Physical Therapist to join our team on a per diem basis. The position would be responsible for providing physical therapy evaluation and treatment to patients in their homes or alternate community settings, within sections of Essex County.
The Community Health & Outreach Specialist is responsible for community outreach to targeted communities in order to link individuals to the VNACJ Community Health Center for primary care and other pertinent services as well as assisting patients in activities related to their healthcare. The Community Health & Outreach Specialist will also participate in community coalitions, relationship building, and outreach events.
