We seek a LGBTQ Transitional Coach to guide those in their transgender journeys.
The LGBTQ Transitional Coach is responsible for providing gender affirming case management assistance to patients who identify as transgender, non-binary or gender non-conforming. This Coach will assist in activities related to each patient's health care, such as navigating the health care system, assisting in appointment scheduling, and providing referrals and resources to increase access to care and other community resources.

The VNA Health Group Children & Family Health Institute is expanding and currently have an opening for an Social Work Manager
The vision of the NJ-InCK is strengths-based with a focus on the families that support NJ FamilyCare children and youth, as well as directly on independent young adults. Goals include improving child health and reducing avoidable emergency department and hospital use as well as of home placement, such as being placed in foster care, juvenile justice detention, or prolonged hospitalization. Further the project aims to create a sustainable pathway for NJ FamilyCare to expand these services throughout the state.
This incumbent will directly supervise a team comprised of Community Health Workers, Family Support Specialist and Child Life Specialist. The team will ensure the children/youth are connected to appropriate Core Child Services including physical and behavioral health care, child care and preschool, school delivered services, maternal child health programs, food and housing supports, legal services, and the child welfare system as needed. This role will not have a direct caseload, but will facilitate the success of the other team members and be responsible for communications with the NJ InCK Care Coordination program, including regular reporting and maximizing use of the data and communications systems which will form the core of both day to day functioning and program improvement and evaluation.

Reports to Covid Community Corp Coordinator for providing outreach, information and education on vaccines, their availability to all targeted communities in Monmouth County. The CHW will work with clients to address social, economic, and health challenges in order to support COVID prevention, testing, and vaccination to better align community members, residents and stakeholders with a healthier place to live, work and play.
