The position would be responsible for providing assessment of patients’ home health care needs and coordination of referrals to home care and hospice. The person in this role will be stationed in either Newark Beth Israel Medical Center, and/or Jersey City Medical Center.
Organizational Overview:
The Greater Newark Health Care Coalition (GNHCC) began in 2008 as a vehicle for response to the closure of two Newark hospitals. As the crisis abated, the participants determined that there was value in the continuation of an organization comprising high-level representatives of Newark area entities concerned with access to high-quality health care, in particular access to primary care in the greater Newark area. Over time, the organization grew to include leaders from hospitals, primary health care, educational institutions, advocacy organizations, behavioral health organizations, visiting nurse organizations, and others. The New Jersey and Newark Health Departments are represented ex officio. Since its inception, GNHCC has undertaken diverse initiatives aimed at addressing health disparities and promoting equitable care to transform and improve health outcomes for our region.
The vision of Greater Newark Health Care Coalition (GNHCC) is for all residents of Newark, East Orange, and Irvington to have a fair and just opportunity to attain their highest level of health. GNHCC is one of four state designated Regional Health Hubs (RHHs) in New Jersey working with both clinical and social service providers to improve patient care and outcomes for Medicaid beneficiaries. In furtherance of this vision, and role as an RHH, GNHCC plays four key roles: it convenes community stakeholders, collects and analyzes data, pilots programs, and advances advocacy and policy.GNHCC focuses on three areas of work:  maternal & infant health, child & adolescent health, and topical regional community health.

The VNA Health Group is seeking a Baylor Registered Nurse to join our dedicated Hospice Team in Bergen County.
Baylor RNs to work two (2) twelve (12) hour weekend shifts per week at manager’s discretion but essentially the shifts are (Sat & Sun- 8:30am-8:30pm with 3- 15 min breaks) . Baylor RNs shall be paid for thirty (30) hours for twenty four (24) hours worked and shall receive the same benefits as RNs, pro-rated to a thirty (30) hour workweek. Paid the 30 hours but are excepted to get your paper work and follow-ups completed within those hours 24-30hrs.
